To place a Checks by Fax order,
you will need to follow these directions:

Please click here to open the Fax
Form for Checks and print the form by clicking on
"File" in the upper left toolbar of your browser
and scrolling down to the "Print" command and
click "Print". Follow computer directions for
printing.

Please fill out the Fax Form and attach
a signed check marked "VOID" if using
the Check By Fax Form.

You will also need to print the following
order form and complete it:
Complete Pre-printed Order Form(6kb)
Finally, fax both the Order Form and the Fax Form to
our eFax number (305) 723-2783. That's it! Your
order will processed and filled in a timely manner.
